| Video Discription | Cushing’s syndrome is a very common syndrome that we encounter in clinical practice. What is syndrome? Syndrome refers to a collection of symptoms and signs. Symptoms is what the patient is complaining of and signs are what we see in the patient. So in Cushing’s syndrome is due to neck cortisol excess. Cortisol is a hormone which is produced form the adrenal gland which are located on top of the kidneys. These adrenal gland produces a hormone called as cortisol. It is a stress hormone which is very vital for our life. But if this hormone is produced in excess quantities, it can produce certain changes which can be recognized as a clinical syndrome. That is by looking at the patient we can make out this patient is producing too much of cortisol. Either he is taking exogenous cortisol, steroids from outside or his glands are producing more of steroids. The more common cause of Cushing’s syndrome is taking exogenous steroids which a doctor is prescribing or the patient is taking by self medication. So how a patient who has Cushing’s syndrome look, they will have moon face, or the face becomes rounded and they can have excess hair on the face, you can have acne or pimples on the face or you can have proptosis or bulging of the eyes, it can be bone feature and most important feature is central obesity, they develop a paunch, most patients who have cortisol excess, they have a paunch or visceral obesity or accumulation of fat within the intraabdominal cavity below the skin, which is mainly around the belly, there can be a buffalo hump, or accumulation of fat behind the shoulders and their hands and legs becomes thinned out and they start getting striate or stretch marks and the striae happens because of protein catabolism. The collagen that is present on the skin is not rebuild very quickly because of cortisol excess and thee is fat accumulation which happens very quickly and both of the leads to a broad purple striae. So these present at the bottom of the abdomen or below the breast or in the upper limb arms or in the limbs. So these are features of Cushing’s syndrome. There is also a sign called as the Lidlle’s sign, where you can pinch the skin and see whether the skin is thick or thin and if the skin thickness is below a particular level then it is very specific for the presence of Cushing’s syndrome, a paper thin skin is a feature of Cushing’s syndrome. Cushing’s syndrome can be present with high blood pressure e recordings. The high blood pressure levels which are there in the blood retain the salt and water. Patients with Cushing’s syndrome can also have diabetes, which is uncontrolled, this is again a feature of Cushing’s syndrome, but it is not clinically manifested.
