Lee Memorial Health System has a program that allows seniors to stay in their home while still receiving the care they need. Dawn Moore, a home health choices case manager at Lee Memorial Health System, says Senior Care Choices is a program that allows seniors to age at home. “Our goal is to really assist families and assist aging folks in living their best lives and aging in place and being the surrogate families for those families who can’t be here. Our aging people are much more savvy. They are much more independent and want to maintain the control over their lives.” Whether it’s managing their pets, taking care of a roof repair, making sure they take their medication, or helping them with their errands, the case worker is there to help the senior with any health or lifestyle concerns they may have. “Everybody has a different plan of care. The needs are very unique to them and their situation,” said Moore. As the client ages and their needs change, their case manager may schedule more frequent visits to help them with different needs. “Whether it’s somebody to take the garbage out or somebody to make sure that they have someone with them all the time so that they’re safe and cared for, or that they need to go somewhere else because this doesn’t meet their needs, we will continue to follow them outside of that into their new home too,” said Moore. With the goal of offering quality of life, case managers take on the role of caregivers, counselors, even drivers. Their mission is to offer comfort to both the client and the family.
